Verum Health collects information you provide directly to us, including name, email address, date of birth, health history, and other information submitted through our intake forms and patient portal. We also collect limited technical information about your use of our website.
Health information collected through Verum Health intake forms and the patient portal is Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). We handle all PHI in accordance with HIPAA Privacy Rule requirements. We maintain Business Associate Agreements (BAAs) with all vendors who may access PHI on our behalf.
We use the information we collect to: process your health intake and facilitate physician review; schedule and conduct consultations; issue prescriptions and fulfill protocol orders; communicate with you about your protocol and orders; and improve the platform.
We do not sell your personal or health information. We share your information only as necessary to provide services — including with our physician medical team, our licensed pharmacy partner, and HIPAA-compliant platform vendors operating under BAAs. We may disclose information as required by law.
We implement technical, administrative, and physical safeguards designed to protect your information. Health intake data is processed through HIPAA-compliant systems. Patient records are maintained in a HIPAA-eligible EHR system.
